PHARADIGM WELLNESS COACH

When you choose to start with a new training programme it is considered best to establish accurate benchmarks of your current body composition and fitness level. The Pharadigm Wellness Coach intervention provides guidance in the selection of biokineticists nationally to facilitate a scientific Pharadigm Fitness Evaluation.

Pharadigm associate biokineticists or “Wellness-Coaches” will:

Conduct 30-minute assessment on Pharadigm members

Analyze a pre-completed Health Screening Questionnaire (Appendix A) and refer members to other medical professionals where required

If appropriate to do so, will conduct a Fitness Evaluation according to scientifically designed protocols. Each evaluation will include measurement of:

Body Mass Index

Body Fat Percentage

Flexibility

Strength

Endurance and

Heart lung fitness or VO2max

Provide the member with an appropriate exercise programme and basic guidance on the use of the programme.

The cost of the evaluation is R189 (excl VAT) and is borne by the member.

PHARADIGM / VIRGIN ACTIVE BENEFIT

FULL Members can activate the Pharadigm / Virgin Active Benefit at any time.

Activated Pharadigm principal and / or spouse members have unlimited access to one Virgin Active club (excluding Classic clubs) free of charge for the duration of his or her Pharadigm FULL Membership whilst complying with the Pharadigm / Virgin Active benefit Compliance Criteria below.

The following payments apply:

Pharadigm Principal or Spouse members can activate at a once off payment of R750.

If the Principal and Spouse wish to activate the Pharadigm / Virgin Active benefit, the second person will activate at a once off payment of R1300 and a Spouse Levy of R50 per month will apply.

If the Principal and / or Spouse opts to have access to all Virgin Active clubs (excluding Classic clubs) a Premier Levy of R70 per month will apply for each premier membership.

If the Principal and / or Spouse opts to have access to a Classic club a once off Virgin Active Classic Upgrade and a monthly Classic Levy will apply for each Classic upgrade. The cost will be confirmed once Pharadigm has received the new Virgin Active rates for 2007.

All rates, activation fees, transfer fees & levies include VAT.

Activation procedure

Please contact the Pharadigm Contact Centre and complete the following procedure:

Complete the Pharadigm / Virgin Active Activation Questionnaire available from the Contact Centre and forward it to Pharadigm on facsimile number provided.

Pharadigm will send you your Pharadigm Letter of Confirmation (LOC). The LOC will provide you with the exact upfront and monthly costs as per your specific membership requirements.

Pay the upfront cost as per the LOC document into the Pharadigm bank account.

Fax proof of payment to facsimile number 031-267 5169 with a cover note with the following details: your Name; Pharos Member Number; Identity Number; Business Telephone Number; Cellular Number; E-mail address and your Unique identity number used on your bank deposit slip: i.e. VAxxxx where x = YOUR Pharos member number

Visit your Virgin Active home club of choice, as you selected in your Pharadigm / Virgin Active Activation Questionnaire to take up the Pharadigm / Virgin Active benefit.

Complete Virgin Active Enquiry card at Club reception.

Produce Pharadigm LOC, ID and Pharos MP card to VA consultant.

Have a guided tour of the Club facilities.

Complete the Pharadigm / Virgin Active application form.

Pay the relevant card fee to Virgin Active as applicable, (e.g. R25 and R60 for any children under the age of 18 years added to the membership). The new Virgin Active access card will allow access to the Virgin Active facility for 10 days.

Upon confirmation of payment from the member, Pharadigm will confirm payment with Virgin Active.

The Pharadigm member will have ongoing access to Virgin Active subject to the Pharadigm compliance criteria as detailed in this document.

Pharadigm / Virgin Active benefit Compliance Criteria

Activated members must access their registered Virgin Active home club (any club in case of Premier members) at least 36 times, per activated member, in any 12 rolling months.

Activated members must be and remain paid up Pharos and Pharadigm members.

Should a member default on the above criteria the member's Pharadigm / Virgin Active membership will be de-activated.

This process is not negotiable. De-activated members will not be allowed to re-activate their membership of the Pharadigm / Virgin Active Benefit.
